Office Policies

Our office adheres to strict policies in order to ensure that our patients receive the best and most efficient care possible.

Scheduling Appointments

To schedule an appointment, please call our office Monday through Friday at 212-535-4611 during office hours, or fill out the “Make an Appointment” online form. Our staff will ask you about the reason for your visit in order to book the most appropriate appointment for your needs.

Canceling Appointments

Please call as soon as possible if you need to cancel or reschedule your appointment. We request that you cancel no later than 24 hours in advance on the business day prior to your appointment as to ensure that the appointment time goes to someone in need.

Late Appointments

In order to ensure that our schedule runs according to appointment time, please arrive to your appointment on time. We value our patients’ time, and therefore strive to stay on schedule; however, we can only do that with your cooperation. If you arrive late for your appointment, we will try to accommodate you as best we can; however, in the event that this happens, you may be asked to wait, or be asked to reschedule your appointment.

Emergencies

Same day appointments are generally available for patients who are experiencing urgent medical problems during normal office hours. If one experiences an emergency outside of office hours, a health care provider who can be reached via our office phone line is on call at all times.

We request that you do not call the emergency line if you are not having an urgent medical problem.

If you are experiencing a serious or life-threatening emergency, dial 911.

Medical Records

If you require a copy of your medical records, you must sign a medical records release authorization form.

Prescriptions

Prescription refills are only processed during our office hours.

If you would like to order your prescription through a mail order pharmacy, you can obtain a mail order form from your insurance provider’s website, and then mail or fax the completed form, along with the written prescription from your practitioner directly to your mail order pharmacy.

Payments

Co-payments are due at the time of the service. We kindly request that new patients complete a credit card authorization form before you are seen at your first appointment. This may be used to cover balances of charges that are not paid or covered by your insurance.
